While injuries and accidents can sometimes happen in soccer, we take every serious incident seriously.

Documenting what happened helps the Club provide appropriate follow-up, support players and families, and help ensure a safe and informed return to play.

This form helps Port Moody Soccer Club document serious injuries and ensure the appropriate follow-up takes place. The information you provide allows our team to understand what happened, support the player and family, and help ensure a safe and appropriate return to soccer when they’re ready.

When should I complete this form?

Please complete a Serious Injury Report when a player:

• Requires medical attention following an injury.
• Is taken to a hospital or urgent care facility.
• Has a suspected concussion or head injury.
• Is unable to continue playing due to a significant injury.
• Experiences a serious incident during a PMSC game, practice, training session or other Club activity.

What happens next?

Once your report is submitted, PMSC will review the information and determine whether any additional follow-up is required. Depending on the nature of the injury, a Club representative may contact you for more information or provide guidance on next steps and return-to-play requirements. Our goal is to make sure players and families feel supported and that players return to soccer safely when they are ready.
